We require all patients to confirm they have read the policy when completing the registration form. We have an FAQ to help with any questions you may have regarding ADHD.
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.
Clicking on the button will lead you to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
Acceptance on to the list is as at the discretion of the Doctor. We may also ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.
Medical treatment is generally available 48 hours from the date of registration unless it’s an emergency. Please contact reception for further information if you require rapid access to treatment. Your medical records may take several weeks to arrive at the Practice.

Step 1: Complete online registration form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Step 2: Upload copies of photo ID and proof of address (and clinical photos/forms only when asked to by a clinician)
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address.
